Please take your time and provide as much detail as possible, but exercise care to not provide details that may reveal your identity unless you wish to do so.

Was someone from IMA Program Administration involved in the incident?
Has the incident been reported to anyone at IMA or anyone at your clinical site?
Has the incident been reported to anyone outside of IMA?
Were there any witnesses?
