Physician Assistant and Pre-PA Internships Abroad

IMA’s pre-PA and PA internships abroad place pre-physician assistant students and PA students in teaching hospitals in Mombasa, Kenya and Cusco, Peru. You shadow physicians and other clinicians and move to a new department each week. Lectures, simulations and community clinics fill the rest of the week. Pre-PA undergraduates, PA students, recent graduates and licensed PAs can apply for stays from two weeks. Your hours are logged and signed, and staff are on call 24/7. Interns who want research experience can also join ongoing research projects led by local medical teams.

IMA’s physician assistant internships for undergraduates put you inside a busy public hospital in Mombasa, Kenya or Cusco, Peru. You spend most of each weekday following the clinicians who run it. You move to a new department each week, so a longer stay lets you see more.

The program is built for pre-PA students who want supervised clinical exposure before they apply. Graduate PA students, recent PA graduates and licensed PAs can apply too.

Admission is by application and a Zoom interview. The sample schedules below show a full program in each region.

IMA was founded by two Johns Hopkins alumni, and its internship program was developed at Johns Hopkins University.

Hospital Internships for Pre-PA Students

Orientation comes first. Local clinicians explain the health system, hospital routines, safety and infection control. Then rotations begin, usually one department a week, for six to eight hours a weekday.

Why Choose IMA for Your PA or Pre-PA Internship?

IMA runs supervised, ethical PA internships that serve the host community as well as the intern. The program is the same whenever you come, but how it fits your year can change. Below, see how summer, gap year and study abroad students use it and how your hours count.

Where You Are Placed

IMA places PA and pre-PA interns in public hospitals and in the community programs it runs with local partners. Your placement depends on the location, your dates and your level of training.

Hospitals

You shadow physicians and other clinicians on the wards and follow patient care under supervision, in departments such as emergency medicine, internal medicine and surgery.

Primary Care

In Cusco, placements lean toward primary care, family medicine and women’s health, where you see how outpatient teams manage common conditions.

Community Health Programs

You join IMA’s community medical clinics and help teach hygiene and health education sessions in local schools.

Specialty Departments

With a longer stay you can spend time in departments that match your interests, such as pediatrics or obstetrics, where the hospital offers them.

Your Program Mentor sets your rotation schedule with you after you are admitted.

Pre-PA Summer Internships

IMA’s pre-PA summer internships fit between spring finals and the fall term. Applications for Summer 2027 are open now. Summer places often fill months ahead, so apply three to six months before you want to start.

Kenya takes interns all summer for 2 to 12 weeks. Cusco runs fixed summer sessions of two, three and four weeks.

Most interns stay four to six weeks. Two weeks covers two departments, and four weeks or more lets you see several.

Many pre-PA students also work a patient care job over the summer. A two- or three-week session at the start or end of the break leaves room for both.

If you want research experience too, say so in your application. Interns can join ongoing research projects led by local medical teams, helping with data collection and literature reviews. See summer dates and fees.

ImageGap Year and Graduates

Gap Year, PA Student and Licensed PA Internships

The Kenya program runs year-round, so gap year students and recent graduates can go in fall, winter or spring. Stays run from 2 to 12 weeks.

Graduate PA students, recent PA graduates and licensed PAs are eligible too. You see how care works in a resource-limited public hospital and join IMA’s community health work.

Note that you pay to attend. This is not a paid job, a clinical rotation for your PA program or a funded fellowship. Pick dates that clear your exams and any job start date.

Pre-PA Study Abroad and Academic Credit

This study abroad option for pre-PA students is built around a hospital, not a classroom. Many interns have used IMA’s PA internships to meet degree requirements or earn academic credit.

IMA works with your school and supplies what it asks for. That can include proof of clinical hours, letters that outline your duties and faculty evaluations.

Talk to your advisor before you enroll, so the approval is settled first. Advisors can start with IMA’s page for university and pre-health advisors.

A Day During Your Pre-PA Internship

Your day starts at the residence with breakfast from the in-house chef. A private program vehicle takes you to the hospital. Most interns are on the wards from about 8 AM to 4 PM.

You spend six to eight hours in your assigned department. You follow the clinicians, watch care and procedures and note questions for your mentor. In the afternoon you may attend a lecture, help at a community clinic or teach a hygiene lesson.

Evenings are for dinner at the residence and time with other interns. Weekends are free for treks or extra hospital time. In Cusco you arrive on a Saturday, have orientation on Sunday and start at the hospital on Monday.

Keep a journal as you go. Record dates, departments, who supervised you and what you learned, but leave out anything that could identify a patient. Those notes feed your CASPA entries and your personal statement.

ImageShadowing Hours

PA Shadowing Hours and PA School Applications

PA programs look closely at the time you have spent around patients, and they sort it into types. CASPA, the application service most PA programs use, lists shadowing, healthcare experience and patient care experience as separate categories.

CASPA defines shadowing as time spent following and observing a healthcare professional at work, preferably a PA. Patient care experience means work in which you are directly responsible for a patient’s care. Read the CASPA experience definitions before you log anything.

Most of what pre-PA interns do at IMA is observation. Report it the way CASPA and each program define it. CASPA notes that program definitions vary, so ask a program directly if you are unsure.

IMA gives you a record of your role, shifts and total hours. Its U.S.-based Medical Director validates it. CASPA asks for a supervisor who can confirm each experience, so note who supervised you each week.

According to the AAPA, many PA programs also require direct patient care experience. It often comes from a job such as EMT, medical assistant or certified nursing assistant. An internship abroad adds to that experience at home rather than replacing it.

Every intern who completes the program receives a certificate of completion. Interns who show professionalism and commitment can request a letter of recommendation. A licensed healthcare professional or senior IMA mentor writes it.

IMA’s PA internships run in two cities: Mombasa on the coast of Kenya and Cusco in the Peruvian Andes. Both have published dates and fees.

Mombasa is IMA’s largest and longest-running site, and the program runs in English, with daily medical Swahili lessons in clinical conversation and terminology. Interns train at Coast General Teaching and Referral Hospital, Kenya’s second-largest public hospital, with 750 beds. You can rotate through emergency medicine, surgery, internal medicine, pediatrics and obstetrics.

In Cusco, the wards run in Spanish, and you need at least two years of Spanish study. The main site is Hospital Nacional Adolfo Guevara Velasco, a 250-bed teaching and referral hospital. Cusco suits interns who want primary care, family medicine and community health.

Every program fee is exclusive of the separate $995 program deposit. The deposit covers screening and processing, your housing reservation, confirmation of your hospital placement and your pre-departure materials.

Fees for three, five and seven weeks are on the program dates and fees page. In Cusco, three weeks costs $4,995.

The fee does not include international airfare, vaccinations, malaria prophylaxis, visa or entry costs or travel outside the program. Optional laundry, the required IMA-branded scrubs and personal spending money are also extra.

IMA offers flexible, interest-free payment plans, and Program Mentors give specific guidance on the plans available for your program dates. IMA also offers need-based and merit-based scholarships. It points interns to outside scholarships, fundraising sites such as GoFundMe and FundMyTravel and financing through Uplift.

Preparing for Your PA Internship Abroad

Once you are accepted, you have 14 days to pay the $995 deposit that holds your place. IMA then assigns your Program Mentor and sends pre-departure materials on vaccinations, visas, travel documents and insurance. Order your IMA-branded scrubs early, and IMA's U.S. team can help you choose flights at no extra cost.
Vaccination and malaria advice should come from your own clinician or a travel medicine clinic. The CDC travel health pages for your destination are the place to start.

You need English proficiency, and Cusco needs at least two years of Spanish. There is no set minimum GPA. Clinical experience helps but is not required for undergraduates.
